Primary sources first

Coverage, claims, coding, and payment statements are mapped to CMS, HHS, Medicare Coverage Database, Medicare Administrative Contractor, or other authoritative code-set and policy sources. Each verified lesson or record shows its review date and links to the source.

Source status

Verified means the published statement is attached to reviewed official sources. Research-stage records are clearly labeled and excluded from the sitemap until code-level review is complete.

Effective dates matter

Code sets, fee files, edits, policies, and appeal rules change. Readers must use the version effective for the date of service, discharge, claim, or decision.

Copyrighted code content

CPT is maintained and copyrighted by the American Medical Association. This site explains workflows and points to licensed/current sources; it does not republish a CPT codebook. Federal HCPCS Level II and ICD-10 resources are linked to their official publishers.

Corrections

Source-backed corrections can be submitted through the feedback form. Editors review the cited source and update the reviewed date when a material statement changes.

Educational boundary

Content simplifies complex rules but does not determine coding, coverage, patient liability, compliance, legal rights, or payment for an individual claim.
