Site inspection checklist

  • A physical location that matches the enrollment record and supports the reported operation
  • Permanent, visible signage appropriate to the business and location
  • Posted business hours that match reported availability and actual operations
  • Accessibility and customer-access arrangements applicable to the supplier
  • Organized business, delivery, maintenance, complaint, and supplier records
  • Current federal, state, and local licenses and permits
  • Supplier enrollment, accreditation, bond, insurance, and ownership documentation
  • Inventory or documented supplier and fulfillment arrangements appropriate to the products
  • Complaint procedures and evidence that staff can use them
  • Protected and retrievable patient records without exposing information unnecessarily
  • Current policy and procedure manuals implemented by the operation
  • Evidence of active business operations, staffing, communications, and service capability
  • Required notices, supplier standards, contact information, and beneficiary-facing materials

Readiness method

Walk the site as an inspector would: compare the entrance, signage, hours, people, inventory or arrangements, records, licenses, and notices with the enrollment and accreditation information. Then sample real workflows so staff can produce the records and explain what happens from intake through delivery, complaint handling, service, and follow-up.
